To get started in services, we would like to know more about you to ensure you receive the best possible care from a provider who best matches your needs. Below are the required documents to get started with our behavioral health services


Complete Child Applications

Copy of Insurance Card, (if applicable)

To see if your insurance will cover your services, please click here or call our office at 217-253-4731 for more information.


Copy of Proof of Income, (if applicable)

The State of Illinois requires RISE Behavioral Health and Wellness to have proof of household income, even if insurance covers your services and/or if you are a foster parent working with a child welfare agency. The proof of income options could include one or more of the following:

  • Last two paycheck stubs from all working members of the household

  • Most recent tax returns from all persons who file taxes living in the household

  • SSI and/or SSDI award letter from all applicable persons in the household

  • Unemployment benefits letter from all applicable persons in the household

  • Illinois state cash assistance statement from all applicable persons in the household

  • Bank statement from all applicable persons in the household

If you do not have proof of income available to you, please contact the office for an alternative form to complete.
